<p>A medical record is a document that includes information about a patient's identity, their treatment program, actions, exams, and services. One of the sheets contained in the medical record is the medical resume sheet. A medical resume is a summary or conclusion detailing the condition, treatment, examination and actions received by the patient. Incompleteness of medical resumes is one of the problems because medical resumes can provide detailed information about what has happened during the patient's hospitalization, which has an impact on the quality of medical records and services provided by the hospital. The purpose of this study was to quantitatively analyze the medical resumes of patients with inpatient surgical cases at Santa Elisabeth Hospital Medan in 2023. The type of research used is quantitative descriptive research. The sampling technique was Simple Random Sampling. The sample in this study was a medical resume sheet for inpatients with surgical cases totaling 95 medical resumes. The instrument used was a checklist sheet. Data analysis using univariate analysis which results in the distribution and percentage of each variable. The results of this study indicate that based on the completeness of the patient identification review is 100%. Based on the completeness of the important report review is 94.73% and the incompleteness of the important report review is 5.27%. Based on the completeness of the authentication review is 100%. Based on the completeness of the recording review is 96.84% and the incompleteness of the documentation review is 3.16%. It is hoped that the hospital, especially the officers who fill out the medical resume sheet, will always maintain and improve the completeness of the patient's medical resume, and improve the discipline of officers in completing medical records by means of good cooperation between nurses and doctors concerned</p>Pestaria SaragihNasipta GintingNur Ayisah Hutabarat

<p>Chronic kidney disease is a progressive disturbance of kidney function that can’t recovered so that need a replacement therapy like hemodialysis. Hemodialysis process can cause physical, psychological and social as well as environmental changes that influence the quality of life. One of the support system that could maximize the quality of life is self efficacy. The quality of life will increase when respondent begin to accept the disease condition and obedient toward treatment process. This research aims to analyse relationship between self-efficacy and quality of life of chronic kidney disease patient that undergo hemodialysis in hemodialysis department of RSUD Embung Fatimah Batam. This study ia a quantitative research that utilized cross-sectional design 48 samples were chosen using total sampling technique. Data was gathered using Chronic Kidney Disease Self Efficacy questionnaire (CKD-SE) and KDQOL SF-36. Data was analyse using chi-square. The result of the study was found that 26 respondents (54,2%) shows high self-efficacy with good quality of life while 22 respondents (45,8%) shows low self-efficacy with poor quality of life (p-value 0,000 < 0,001). As conclusions is there is a relationship between self efficacy and quality of life of chronic kidney disease patient who undergo hemodialysis. <p><em>Critical thinking is the ability to think rationally that aims to understand something important in providing quality nursing care. The phenomenon that occurs is that there are many students who are still not active in critical thinking because they are afraid to express their opinions, ideas and are not confident in their abilities. This research was conducted to find out the factors related to the motivation of critical thinking of students of the Nursing Study Program. Methods: This study is a correlational descriptive research with a cross-sectional approach. The instrument in this study uses a questionnaire. The sample in this study amounted to 123 people with a total sampling technique. Results: The data processing of this study was processed using the spearman test. The results of this study show that the factors that affect the motivation of nursing students to think critically are self-efficacy (p value = 0.000), time management (p value = 0.000), and critical thinking ability (p value = 0.000). <p>Solubility is a problem in a drug characteristic, because drugs with low solubility will cause low bioavailability and effectiveness. Simvastatin is one of the drugs for hypercholesterolemia that has low solubility, so it needs modification to increase its solubility. Self Nano-Emulsifying Drug Delivery System (SNEDDS) is one of the modifications of drug delivery system that can increase the solubility. SNEDDS is formulated with VCO as oil, tween 80 as surfactant and PEG 400 as cosurfactant. The purpose of this study is to obtain the formula with the best characteristics of the PEG 400 variations of 1g, 2g and 3g. SNEDDS preparations were carried out organoleptic tests, phase separation tests, % transmittance tests, pH tests, viscosity tests, emulsification time tests and characterization include of droplet size, polydispersity index and zeta potential was measured by Particle Size Analyzer (PSA). Based on the tests that have been carried out, the best formula of SNEDDS simvastatin was obtained at a concentration of PEG 400 1g, with the characteristics in order, namely in the form of a slightly thick liquid with a transparent yellow color and a distinctive smell of VCO, no phase separation was formed either in the centrifugation method or during storage of 28 days, the transmittance % was 97.966%, pH 8.886, viscosity was 28.206, and the emulsification time was 57.176. The results of characterization using PSA instruments were 16.323 nm, 0.266 and -13.61 mV, respectively. <p>Telang leaf is a plant species (<em>Clitoria ternatea </em>L.) and belongs to the <em>Fabeaceae </em>family. In this study, the antimicrobial activity of the ethanol extract of butterfly pea leaves (<em>Clitoria ternatea </em>L.) will be tested against microbes that cause skin infections. The method used in the antimicrobial activity test was the diffusion method using paper discs against the bacteria <em>Staphylococcus epidermidis </em>ATCC 12228, <em>Propionibacterium acne </em>ATCC 11827 dan jamur <em>Trichophyton mentagrophytes </em>ATCC 52016, <em>Malassezia furfur </em>ATCC 14521. The concentration of the ethanol extract of telang leaves used was 30%, 40%, and 50%. As a positive control for antibacterial using clindamycin, antifungi using ketoconazole and negative control using distillate ethanol. The test results showed that the ethanol extract from telang leaves has potential as an antimicrobial against bacteria and fungi. The measurement results of the largest inhibition diameter were at a concentration of 50% against <em>Staphylococcus epidermidis </em>ATCC 12228 obtained an inhibition zone of 10,33 mm, <em>Propionibacterium acne </em>ATCC 11827 obtained an inhibition zone of 10,03 mm, and the fungi <em>Trichophyton mentagrophytes </em>ATCC 52016 obtained an inhibition zone of 7,23 mm, <em>Malassezia furfur </em>ATCC 14521 obtained an inhibition zone of 8,23 mm</p>Masayu AzizahSiska AnandaReza Agung Sriwijaya

<p>The maternal mortality rate is still high, especially in countries with low and lower middle income. There are three top causes of maternal death, namely eclampsia, bleeding and infection. Maternal and Neonatal Health introduces international standard method, namely active management of the third stage in an effort to prevent bleeding in stage III. The results of peer review of normal Childbirth care in Wonosobo found that midwife’s compliance in implementing active management in stage III from 20 respondents was only 63%. The aim is to determine the compliance of midwife’s in implementing active management in stage III of Normal Childbirth Care at Puskesmas Sukoharjo I and Sukoharjo II, Wonosobo Regency. Quantitative descriptive research with 30 respondents. Data was taken using checklist containing age, education, length of practice and implementation of active management in stage III which was adopted from the skills checklist I of OSCA book. The data was analyzed using frequency distribution analysis to describe characteristics of respondents and midwife’s compliance with carrying out active management in the third stage of Normal Childbirth Care. The results showed that 27 (90%) respondents had level of compliance in implementing active management in the third stage, while 3 (10%) respondents didn’t comply. The conclusion is that there are midwife’s who are still disobedient in implementing Active Management in Stage III of Normal Childbirth Care. <p>Tuberculosis became a deadly infectious disease in the world. The disease is mostly contributed by adults, with the remainder donated by children. As another problem arises, people are resistant to anti-tuberculous drugs. These conditions require steps in accurate diagnosis so that proper treatment can be given and no fatal problems occur later. One way of diagnosing tuberculosis is by examining Molecular Quick Tests because of its high sensitivity and specificity and its results can be known faster. The purpose of this study illustrates the results of TCM examination on pulmonary TB suspect patients at the Public Health Center Ngadirejo, Temanggung. The research method is a descriptive quantitative with cross sectional approach to observing secondary data from TCM examinations on suspected pulmonary TB patients examined in January–November 2022. The results of the study showed that of 463 patients with suspected pulmonary TB who underwent TCM examination, 455 (98,27%) had negative MTB patients, 7 (1,51%) sensitive rifampicn MTB patients, and there was 1 (0,22%) patient who was detected to be rifampicin resistant. The conclusion of TB disease is still a serious problem, in addition to active TB problems, latent TB problems, MDR-TB and XDR-TB problems. <p>Smoking habits have become a lifestyle for all groups from old to young, even among children. The prevalence of smokers in Indonesia is quite high with 69.1 million adult smokers. The environment of cigarette smoke causes several diseases such as heart disease, lung cancer, and the risk of laryngeal neoplasm. Supporting factors for someone to choose which actions are good and bad, especially smoking behavior is influenced by knowledge. The aim is to determine the relationship between the level of knowledge of the dangers of smoking and smoking behavior at the UNSIQ Male Student Islamic Boarding School. The research method uses an observational correlation design with a Cross Sectional approach. The study population was 40 students, the research sample with total sampling. Data analysis using the Spearman-Rank test to determine the relationship between the level of knowledge of the dangers of smoking and smoking behavior. The results of this study obtained 13 respondents with good knowledge, 15 moderate, and 12 low, while the behavior of respondents as many as 20 respondents had high smoking behavior and 20 had moderate smoking behavior, and the p-value results were 0.100> 0.05. <p>Stroke is a medical condition that requires immediate treatment because it can cause permanent brain damage or even death if not treated quickly. Delays in medical treatment can lead to increased rates of morbidity and even death. Therefore, family knowledge about early detection of stroke is needed. Families who know about early stroke detection will be able to take patients to hospital more quickly for immediate treatment, thereby reducing the risk of permanent disability. The aim of this study was to analyze the relationship between family knowledge about the FAST method and family behavior in the initial treatment of stroke at the Antonio Baturaja Hospital polyclinic. The method used in this research was cross sectional, with a total of 33 respondents taken using a purposive sampling method from families whose family members received treatment at the Neurology Clinic at Antonio Baturaja Hospital. The research results showed that 63.6% (21 respondents) had a high school education, with a knowledge score of 97% (32 respondents) had sufficient knowledge and a score for family behavior of 100% (33 respondents) was declared good. This study used the Spearman correlation test where the p value was 0.039 and r 0.326, so there was a significant relationship between family knowledge and family behavior in early stroke management. The conclusionof this study is that there is a significant relationship between family knowledge regarding the FAST method and family behavior in early stroke management with a positive correlation, meaning the higher the knowledge, the better. <p>A hospital is a health service institution that provides complete individual health services that provide in patient, out patient, and emergency services. Hospitals can be established by the Central Government, Regional Government, or the private sector. Medical records are documents that contain patient identity data, examinations, treatments, actions, and other services that have been provided to patients. Medical records aim to improve the quality of health services, provide legal certainty in the implementation and management of medical records, ensure the security, confidentiality, integrity, and availability of medical record data, and realize the implementation and management of digital-based and integrated medical records. The purpose of this study is to analyze the completeness of medical record files of dengue fever patients on patient identity sheets, anamnesis, informed consent, medical resumes at Santa Elisabeth Hospital Medan in 2023.The type of research used is descriptive research. The sample in this study is as many as 76 medical records. Sampling was carried out using the simple random sampling method. The instrument used is a checklist sheet. The results of the study obtained a percentage of completeness for the patient's identity medical record of 88.2%, complete files, 89.5% anamnesis, informed consent of 100%, medical resume of 100%. The cause of incompleteness is due to a lack of precision and discipline in filling out electronic medical record files. <p>Health developmentis part of national development aimed at increasing awareness, willingness and ability to live healthily for everyone in order to achieve the highest level of public health. Efforts to realize this level of health can be in the form of traditional health services (Traditional Health). The aim of this researchis to look at the factors related to the quality of traditional health services at the Palembang City Health Center in 2024. The method in this research is a quantitative method with a cross-sectional approach. The research was conducted at the Palembang City Health Center with a population of 41 and the sampling technique was total sampling. Statistical tests use the Chi Square test. Multivariate analysis in this study used multiple logistic regression analysis. The research results showed that there was a relationship between age (p value=1.000), education (p-value=0.518), employment status (p-value=0.144) and length of service (p-value=0.588), a relationship between technical competency (p-value=0.015), service effectiveness (p-value=0.002), Service comfort (p-value=0.012) Service security (p-value=0.020) Human relations (p-value=0.001), Service continuity (p-value=0.000), Service time (p-value=0.012) with the quality of traditional health services at the Palembang City Health Center in 2024. The continuity of service variable is the most dominant variable because the p-value is 0.003 (<0.05) with OR=30,891. It was concluded that there is no relationship between age, education, employment status and length of work with the quality of traditional health services at the Palembang City Health Center in 2024. There is a relationship between technical competency, service effectiveness, service comfort, service security, human relations, continuity of service, service time and the quality of health services traditional services at the Palembang City Health Center in 2024. <p>Elderly people who have unhealthy behaviors such as smoking habits, have the perception that the cost of checking burdens the family and are lazy to check their health because they feel healthy, will affect the ability of the elderly to access health information. This study uses a quantitative method with a cross sectional approach, which aims to determine the relationship between health literacy and illness perception in elderly Diabetes Mellitus. The population in this study was 150 with a sample of 117 elderly people with diabetes mellitus who participated in filling out the questionnaire. The HLS-EU-Q16 (Health Literacy Survey Questionnaire) questionnaire was used to measure health literacy and B-IPQ (The Brief Illness Perception Questionnaire) to measure illness perception. The sampling technique of this study used accidental sampling with data analysis using the spearman rho test to determine the correlation of two variables. The results showed that most respondents had sufficient health literacy and some respondents had positive illness perception. Health literacy correlates significantly with illness perception (p < 0.05) and the direction of the correlation is positive, meaning that the higher the health literacy, the higher the Illness Perception. The strength of the correlation of r = 0.571 includes moderate criteria. <p>Breast cancer cases in Indonesia were 58,256 cases or 16.7% of the total 348,809 breast cancer incidents reaching 42 cases per 100,000 population, with an average death rate of around 17 cases per 100,000 population. Research design with quasi experimental one group pre-test and post-test. The population in this study was 25. Samples taken (total sampling). A total of 25 samples. The results of the univariate analysis before counseling was given, the average student score was 5.76, standard deviation 1.128, while after counseling the average student score was 7.96. and standard deviation 1,013. Results of bivariate analysis, the average value of pre-test knowledge is 5.76, while the average value of post-test knowledge is 7.96. It can be seen that the increase in the average value between knowledge before and after being given counseling is 2.2. The results of the wilcoxon signed ranks test statistic showed a p value of 0.000. <p>Data from the World Health Organization (WHO), 15-20% of maternal deaths are directly or indirectly caused by anemia, on the other hand, anemia is also related to maternal morbidity. This study aims to determine the compliance factors for TTD consumption in pregnant women, using a cross sectional design. The population of pregnant women in Tanjung Enim Subdistrict is 81 pregnant women and a sample of 81 samples is needed using a simple random sampling technique. Univariate analysis results show that of the 81 respondents, 45 (55.6%) of TTD consumption compliance were non-compliant, 36 (44.4%) were compliant, 47 (58.0%) had poor knowledge, 34 (42.0%) had good knowledge. Negative attitudes 43 (53.1%), positive attitudes 38 (46.9%). Bivariate analysis chi square statistical test obtained a p value of 0.000, which means there is a significant relationship between knowledge and compliance with TTD consumption. The p value is 0.000 which means there is a significant relationship between attitude and compliance with TTD consumption. <p>Dug wells are the main source of clean water supply for residents living in rural and urban areas in Indonesia. Dug well water is very easily contaminated by bacteria originating from wastewater or human waste. The aim of the research is to find out the factors that influence the presence of ecoli bacteria, using a cross sectional research design. The population in the study was 116 dug wells with the number of samples needed being 89. Samples using a random sampling technique. The results of the univariate analysis showed that there were 89 respondents. 38 (42.7%) respondents were there, 51 (57.3%) were not there. 45 (50.6%) respondents whose Sepitctank Distance did not meet the requirements met the requirements as many as 44 (49.4%). 40 (44.9%) respondents whose physical condition of the dug well did not meet the requirements were that the physical condition of the dug well met the requirements, as many as 54 (60.7%). Bivariate analysis Chi Square statistical test of septic tank distance, waste accumulation distance and physical condition of dug wells obtained a p value of 0.000. Conclusion. <p>Coverage of clean and healthy living behavior in Indonesia is still low because the percentage of schools implementing clean and healthy living behavior is only 35.8%, while the national target for educational institutions is 70%. The coverage rate for clean and healthy living behavior in Indonesia has not yet reached the national target for educational institutions. The aim of the research is to determine factors related to clean and healthy living behavior in elementary schools. The research design used cross sectional. The population of all students in grades IV and V at SD N 49 Ogan Komering Ulu Regency was 109 students. The total sample is 85 Sample. Univariate analysis results show that of the 85 respondents, 51 (60.0%) respondents did not practice clean and healthy living behavior, while 34 (40.0%) did clean and healthy living behavior 64 (75.3%) poor knowledge, good knowledge 21 (24.7%). 53 (62.4% poor attitude, good attitude 32 (37.6%). 59 (69.4%) had a poor teacher role, 26 (30.6%) had a good teacher role. Bivariate analysis using the chi- square statistical test, knowledge p value 0.002, attitude p value 0.000, teacher's role p value 0.001 Conclusion there is a significant relationship between knowledge, attitudes and the role of teachers and Clean and Healthy Living Behavior (PHBS) at State Elementary School 49 Ogan Komering Ulu Regency</p>WidiaEva Yustati
